QUALIFICATIONS: Masters’s Degree in counseling, psychology, social work or a related field from an Accredited University
Ability to perform group instruction & activities
Good organizational & documentation skills
Ability to communicate effectively with clients and/or referral sources
Proficient writing skills
DSM IV knowledge
Knowledge of community agencies
Experience preferred working with the elderly
Effective planning skills, concurrent documentation skills Computer Skills
DUTIES AND RESPONSIBILITIES:
1)Conduct individual, group and/or family therapy to patients in nursing home
2)Perform initial assessments, intakes and prepare documentation of such for client charts
3)Prepare with client a treatment plan to include goals
4)Document progress of client to goals as outlined in the Treatment plan
5)Input data into computer client charts
6)Present cases to psychiatrists regarding client’s medicine
7)Work closely with Geriatric Services Assistant and nursing home staff in relation to client care.
8)Maintain billable and productivity standard as set forth by the Center.
9)Provide Crisis Intervention as needed
10)Sign in & out of nursing home, coordinate referrals with nursing home and communicate information that will assist
nursing home staff with client.
11)Participate in various meetings to include departmental, divisional or other meetings as required by the Manager and/or Division Director.
12)Complete billing documentation in a timely and accurate manner.
13)Conduct follow-up with clients, nursing home staff and family members.
14)Other duties as assigned by Manager and/or Division Director.
